Under the Rhode Island Access to Public Records Act, §38-2-1 et seq., I am requesting an opportunity to inspect or obtain copies of public records that [Describe the records or information sought with enough detail for the public agency to respond.  Be as specific as your knowledge of the available records will allow. But it is more important to describe the information you are seeking.

Please send me all documents,emails,phone call records,interoffice memos,pertaining to my formal public charges  against Brookdale Cone in Coventry,R.I. about my being left in Rm 284    that was vibrating  and damaging my health.I  would like to receive the report of the investigation  that your dept (R.I. Dept. of Health) stated would be done about my formal public charges against Brookdale Cone of Coventry,R.I.

hehe If there are any fees for searching or copying these records, please inform me if the cost will exceed $______.  However, I would also like to request a waiver of all fees in that the disclosure of the trequested information is in the public interest and will contribute significantly to the public’s heunderstanding of the security of the elderly in elderly residence institutions ln Rhode Island.
